Required Clearances

The information and links below pertain to individuals who are seeking a position with at Central Susquehanna Opportunities.  The FBI Criminal History Report is required only if you are seeking a position working with youth or in a school district. You may click on the following links to go directly to the registration pages for Act 34,Act 151 and Act 114 clearances.  If you are unable to access the documents you may go to your local CareerLink for a paper copy.


Act 34The Pennsylvania State Police Request for Criminal Records Check (Fee:$22)

The PA State Police uses a web-based computer application for which you must use a credit card to pay the fee. The results will not be mailed. You must return to the ePatch site and print your criminal history check results.



Act 151Child Abuse Report (Fee: $10)

Complete Section No. 1 of Form CY-113 completely and accurately.  Enclose a $10 money order and mail the form to Childline and Abuse Registry,Department of Public Welfare,PO Box 8170,Harrisburg PA 17105-8170. It will take approximately 4-6 weeks for the form to be processed.  The results are mailed directly to you.
